Participation waiver

I acknowledge that participation in Intergalactic Adventures programming — including entry into the mobile space and science museum trailer and use of its interactive exhibits — is voluntary.

I understand that Intergalactic Adventures carries insurance and operates as a certified school vendor, and that staff supervise all exhibit stations during a visit.

I grant permission for photographs or video taken during the visit to be used for educational and promotional purposes, unless I opt out below.

I confirm that the information provided in this form is accurate and that I am the parent or legal guardian of the participant named above.
